Medical Records are the foundation of most legal consultant activities in explaining the significance to attorneys, medical records are often the most important aspect of medical legal cases. The Medical Record Review process involves the assessment of patient medical information. The records
would be sorted and arranged into sub-sections such as progress notes, laboratory reports, specialist consultations, diagnostic examination reports, emergency medical records, radiographic and imaging reports and operative notes.
Medical Record Review is done by subpoenas, affidavits and Deposition by Written Questions. With new HIPPA rules it is often necessary to provide secondary documentation, often in the form of an authorization, but not always. Medical records will often include more than just notes. X-ray / MRI films (mostly on electronic media now) are also often obtained and copied.
