Complete the Vendor Application.

 Registration deadline is November 16, 2019.

Include photos and/or a catalog of the items to be exhibited. (Photos will NOT be returned.)

Include a raffle donation item or gift certificate with a minimum value of $25. This will be used as a raffle prize during the Event. Please include your business card and detailed description of the donated item—this will be included with the raffle prize.

Event Location: Our Lady of Fatima Academy – McGowan Hall, 105 N. La Esperanza, San Clemente, CA 92672 Event Date: Friday, December 6, 2019

Boutique Hours: 9:00 AM – 4:00 PM

Set Up: Thursday, December 5th / 4:00 PM – 8:00 PM Tear Down: Friday, December 6th / 4:00 PM – 6:00 PM

Vendor Space: Each space accommodates a 4’ x 6’ rectangular table (to be provided by Vendor); chair(s) (provided by OLF); optional electrical outlet (provided by OLF, at extra cost).

Pricing:

 Vendor Space $75 inside, $65 outside  Electrical Outlet (optional)

o Vendor provides extension cord. $25

 Catered Lunch (optional) $35  Payment Methods:

Deadlines: Registration deadline is November 16, 2019. To save your space, all paperwork, payment, and raffle donations must be received by November 16, 2019.

Lunch: Vendors may opt to purchase a catered lunch or bring his or her own. Notes:

1. Due to a limited number of vendor spaces, applications will be accepted on a first-come, first-serve basis. We can discuss availability and space placement during registration.

2. Completed applications and payment must be received before space is secured. 3. OLF will assign vendor spaces as applications arrive.

4. OLF reserves the right to limit the number of vendors selling similar items.

5. Vendors must bring his or her own table. Each space accommodates one 4’ x 6’ rectangular table. 6. If Vendor has opted to pay for an electrical outlet, he or she must bring extension cord(s). 7. Vendors are not required to purchase a lunch ticket.

8. If Vendor sells tangible personal property of any kind ordinary subject to sales tax, Vendor is required to hold a valid seller’s permit. Permits must be provided to OLF with your permit number and a copy of your sales tax permit. For more information, contact the State Board of Equalization.

T E R M S A N D C O N D I T I O N S

I (Vendor) understand and agree to abide by the following OLF terms and conditions of use:  Event Date: December 6, 2019, 8:00 AM to 4:00 PM.

 Event Location: Our Lady of Fatima Academy – McGowan Hall; 105 N. La Esperanza, San Clemente, CA 92672.

 Payment: Vendor’s total cost of space and raffle donation are due by November 16, 2019 (for regular pricing) to ensure your space is reserved. Payments not received by such date can result in your space being given to the next waiting vendor.

 Vendor must remain on site from 9:00 AM to 4:00 PM on the day of the event.

 Set up: McGowan Hall will be open to Vendor for set up on Thursday, December 5th from

4:00 PM until 8:00 PM. All set up must be complete by 8:00 PM on Thursday evening— Vendor will not be permitted to set up on the morning of the event. All merchandise will be locked up and secured in McGowan Hall prior to the event. If you are an outside vendor, you may choose to set top the morning of the event.

 Pets are not allowed in the event location at any time, including during set up, event hours, and tear down.

 Tear down: Vendor must pack and remove all items and debris by 6:00 PM on day of event.  Vendor may not bring alcoholic beverages into the event location.

 Smoking is not allowed.

 OLF reserves the right to refuse Vendor’s application or limit the sale of certain items it if it believes Vendor’s merchandise is inappropriate to the spirt of the family-oriented event.  OLF reserves the right to change the event layout or space locations if it deems necessary.  OLF reserves the right to remove any persons not complying with the above regulations--

without a refund.  Cancellations:

o No refunds of space rental or lunch tickets will be made after November 23, 2019 unless the space can be filled from the vendor waiting list. There will be a $10 charge for processing refunds.

o OLF is not responsible for unforeseen and extreme circumstances out of our control such as fire, rain, or any other inclement weather. OLF spends substantial sums in the preparation of this event and refunds cannot be made if the Event is cancelled in part by any such circumstance.
