S.M.A.R.T. Treatment Planning

z Treatment plans are essential (and required) tools

problems identified in the client assessment, defines and measures interventions in their care and provides a measure for client’s progress in treatment.

Elements of Performance require treatment plans that include the following:

z The frequency of care, treatment, and services

z Objectives are sufficiently specific to evaluate the client’s progress and

Goal

Objective

z A statement in specific and measurable terms that describes what

the patient will be able to do as a result of your methods and the the patient will be able to do as a result of your methods and the patient’s efforts.
