(the Choice Plan and the Choice Plus Plan), as well as access to many

Preventive care services
Preventive care services include wellness exams, well-child exams, physicals, mammograms, flu shots and other immunizations. Women’s preventive health services are also covered, including well-woman visits, gestational diabetes screenings and more.

The Retirement Board of the County Employees’ and Officers’ Annuity and Benefit Fund of Cook County and ex officio for the Forest Preserve District Employees’ Annuity and Benefit Fund of Cook County reviews the health benefits plan structure at least annually. All aspects of the health benefits program are subject to change at any time at the discretion of the Retirement Board. HOW THE CHOICE PLAN WORKS
You have the freedom to use any doctor or hospital in the network. Note: If you do not use the network, the plan will not cover the cost. This means you
will be responsible for the entire cost of the service.
You do not need to choose a primary care provider.
But a primary care provider (PCP) can be helpful in managing your care.
You do not need a referral to see a specialist.
See any doctor, including specialists, without referrals.

Convenience care clinics
Can’t get to your doctor’s office but your condition is not urgent or an emergency. Convenience care clinics are often located in retail stores and offer services for minor health conditions. Find in-network clinics online or by calling customer care.

You will pay a co-payment for doctor visits.1, 2 A co-payment is the fixed amount of money you have to pay each time you see a doctor.

If you reach the out-of-pocket limit, the plan will pay 100% of covered services for the rest of the plan year. You may be required to receive approval for some services before they can be covered by your plan.
HOW THE CHOICE PLUS PLAN WORKS

Take a personalized health assessment.
When you register or log in to myuhc.com, you can complete the personal health assessment. You’ll get your results right away, including useful tips and tools to help you live healthier.
Work with an online and telephonic wellness coach.
After you take the health assessment, we may recommend an online wellness coaching program. If you want more personal coaching, you can receive telephonic wellness coaching. Chatting online with a nurse is a fast, easy way to get answers to your health questions. You can also request a record of your chat session for future use. All chat sessions are private and secure.

You can use tools like the symptom checker to check symptoms, track and record your blood pressure and check your risk for certain health conditions like cancer, heart disease and diabetes.

If you need to speak with a registered nurse
myNurseLineSM services is a personal service that connects you with registered nurses who can help answer your health care questions. This service is available 24/7. The nurse can:
• Answer questions about an illness or injury • Provide support on managing a chronic condition
• Help you find high quality network doctors and scheduling appointments • Explain medications, drug interactions or medication alternatives
• Inform you about preventive care
You can access myNurseLine with the Health4Me app or you can call 1-800-846-4678.
If you need care management
Through care management, we’ll work with your doctors and other health care providers to help you understand your treatment options if you need care. We’ll also let you know about special programs to help you make informed health care decisions or to help manage a condition.

Behavioral health support is available any time you need it. You can visit
