Fund you have the choice of two health care plans from UnitedHealthcare

(the Choice Plan and the Choice Plus Plan), as well as access to many

additional tools and programs to improve your health. We hope you use

this guide to learn about your benefits and the many great services

available to you.

Preventive care services

Preventive care services include wellness exams, well-child exams, physicals, mammograms, flu shots and other immunizations. Women’s preventive health services are also covered, including well-woman visits, gestational diabetes screenings and more.

Health care providers who are in our network have agreed to charge lower prices, which helps lower your health care costs. There are two ways to find out if your local doctor, clinic or hospital is in our network.

This information is a brief, general description of your coverage; it is not a contract and does not replace your Certificate of Coverage/Summary Plan Description. For a complete list of your coverage, including exclusions and limitations relating to your coverage, please read your Certificate of Coverage/Summary Plan Description. If descriptions, percentages, and dollar amounts conflict with official benefit coverage documents, the official benefits coverage documents prevail.

The Retirement Board of the County Employees’ and Officers’ Annuity and Benefit Fund of Cook County and ex officio for the Forest Preserve District Employees’ Annuity and Benefit Fund of Cook County reviews the health benefits plan structure at least annually. All aspects of the health benefits program are subject to change at any time at the discretion of the Retirement Board. The benefits described in this Open Enrollment Guide, including plan design and rates, have been approved for the 2015 plan year only and should not be construed as a guarantee of future benefits.

HOW THE CHOICE PLAN WORKS

You have the freedom to use any doctor or hospital in the network. Note: If you do not use the network, the plan will not cover the cost. This means you

will be responsible for the entire cost of the service.

You do not need to choose a primary care provider.

But a primary care provider (PCP) can be helpful in managing your care.

You do not need a referral to see a specialist.

See any doctor, including specialists, without referrals.

A national network to help

Convenience care clinics

Can’t get to your doctor’s office but your condition is not urgent or an emergency. Convenience care clinics are often located in retail stores and offer services for minor health conditions. Find in-network clinics online or by calling customer care.

You will pay a co-payment for doctor visits.1, 2 A co-payment is the fixed amount of money you have to pay each time you see a doctor.

HOW THE CHOICE PLUS PLAN WORKS

SUPPORT TO REACH YOUR WELLNESS GOALS

Here are just some of the programs and services to help you maintain your health and reach your wellness goals. These are included at no extra cost to you and your covered dependents.

Take a personalized health assessment.

When you register or log in to myuhc.com, you can complete the personal health assessment. You’ll get your results right away, including useful tips and tools to help you live healthier.

Work with an online and telephonic wellness coach.

After you take the health assessment, we may recommend an online wellness coaching program. If you want more personal coaching, you can receive telephonic wellness coaching. Our wellness coaches will work with you to create a personalized action plan, and will be in contact with you by phone to help keep you motivated. Wellness programs include:

Safely store your health history.

The Personal Health Record is your health history, a medical library and a health care organizer rolled into one secure place. You can print a summary to share with your doctors.

Chat online with a nurse.

Chatting online with a nurse is a fast, easy way to get answers to your health questions. You can also request a record of your chat session for future use. All chat sessions are private and secure.

Check your symptoms and much more.

You can use tools like the symptom checker to check symptoms, track and record your blood pressure and check your risk for certain health conditions like cancer, heart disease and diabetes.

PERSONAL CARE WHEN YOU NEED IT

You’ll get personal attention if you need help finding the right care or managing a complex health condition. Here are just some of the clinical programs and services that are available. These are included at no extra cost to you and your covered dependents.

If you need to speak with a registered nurse

myNurseLineSM services is a personal service that connects you with registered nurses who can help answer your health care questions. This service is available 24/7. The nurse can:

• Answer questions about an illness or injury • Provide support on managing a chronic condition

• Help you find high quality network doctors and scheduling appointments • Explain medications, drug interactions or medication alternatives

• Inform you about preventive care

You can access myNurseLine with the Health4Me app or you can call 1-800-846-4678.

If you need care management

Through care management, we’ll work with your doctors and other health care providers to help you understand your treatment options if you need care. We’ll also let you know about special programs to help you make informed health care decisions or to help manage a condition.

If you need help with everyday challenges, including serious issues

Behavioral health support is available any time you need it. You can visit

liveandworkwell.com to learn more. You will have confidential help for:

• Depression, anxiety or stress • Relationship difficulties

• Alcohol and drug abuse recovery • Coping with grief and loss and more

If you are dealing with the challenges of disease

The Disease Management Program will connect you with specially trained nurses and experts who can provide support for you and your family to deal with a condition or disease. Please see your health plan documents to learn what programs are available to you.

The Diabetes Management Program can connect you with specially trained nurses that and experts that can help you reduce the life threatening risks of diabetes. If you choose to participate, you will:
