Artwork based on the original design by Lin Tobias/La Bella Design

(3)

The HELP Gippsland Local Service Directory has been developed by the GRICS Consumer Reference Group in response to the request by local cancer patients to be able to find out what services are available to assist them within their local area.

In addition this Directory allows you to:

♦ Document your personal and medical information

♦ Keep appointment cards in the handy plastic sleeve

♦ Store current pathology request slips, recent results or medication list in the plastic pocket

♦ Track your travel and accommodation stays.

This is the first in a suite of information products that will be developed to meet the needs of local cancer patients.

GRICS would like to extend thanks to the members of the GRICS Consumer Reference Group and those other community members who have assisted in the creation, development and review of this document.

(4)

Services have been listed according to Local Government Areas (See map below).

Private Practitioners have not been included. Listings of private practitioners can be found in community directories and Yellow Pages. For some services the following may apply:

♦ Costs associated with the service;

♦ Appointments may be required;

♦ Hours and days of operation may vary;

♦ Waiting periods;

♦ There may be an eligibility criteria; or

♦ There may be region restrictions.

Please check with the Service Provider to determine whether any of the above apply.

Every effort has been taken to make sure that the details in this directory are correct. Inevitably some details may have changed or new services may have started. If you notice any mistakes or omissions please contact Enza Wright on 5128 0138 or [email protected]
